Table with mysql data

Hello to everyone,
i have a little problem, i cant put my sql data in the right position of the table.
i have a mysql table (Notas)that as:
NumMec
Ano
Curso
Disciplina
Modulo
Nota
In the attachement you can see what i want to do. the trick is i want to select data only for one user so the table will have only that user info.
i apreciate all the help and sorry for the bad english.
