Form submissions are blank in email notification

This is happening on several client sites that are all running 5.6.3.3
Using the c5 form block - the form email notification is blank with only the link to view in dashboard. The actual submission in Form Results does have all the information.
I don't believe there is any additional code applied to this block. This has been working until the upgrade to 5.6.3.3
Any help is greatly appreciated.
