Tiered pricing issue

There may be an answers for these issues somewhere on the forums but I haven't come across anything but if there is would someone send me the posts?
Okay so I have a client who we are doing a website for her t-shirt company and we are using the tiered pricing which is a great feature. She has a set fee of $10 total (not by item) for orders that are 1-23 items and anything 24-up do not have that charge.
My questions:
Is there a way to create an additional price if someone chooses 1-23 items without the customer choosing it in the customer choices?
She is also selling XXL, XXXL and XXXXL t-shirts for extra, how can I set these so someone can choose more than just one XXL, XXXL or XXXXL?
Any help would be greatly appreciated.
