After you download the Add-On, unzip the file and you will need to place the unzipped folder and all files it creates into the /blocks folder at the root of your site.

Don't place it in /concrete/blocks make sure it's in the root /blocks folder location. Then proceed to the Dashboard and select Add Functionality. You will see the new block at the bottom of the list ready to be selected for install.
