Form in Default section of Page Type

I have a form in the default section of a custom page type. When new pages are based on that page type, I can't preview or edit the form as I get 'Invalid Permissions' even though user is in Administrator group. See attached screen shot.

I deleted the block, both in Page Type defaults, and all pages with that type. Manually re-entered the form this time (as opposed to copying an existing block), then did 'Setup on Child Pages' and all is well. No more permissions errors, and the form can be edited as desired.
