Interactive Form. Changes Based On Answers.

Can anyone make a recommendation, or point me in the right direction on how I can achieve the following. I would like to make a form, that serves a custom set of questions, based on responses to other questions. For example. If you were to ask a question, and the optional answers were A, B, C, and D, based on the answer given, I would want the form to give a custom set of questions. So, if the user selected "A" as the answer, a unique set of questions would appear on screen for them to answer. If a user selected "B" as the answer, a different set of questions would appear only pertaining to answer "B". Any suggestions on how this can be achieved would be appreciated. I know this can be done with jquery, but I'm hoping there is an add on that has this fuctionality.

ProForms can do this with "conditional" options set per question. Not all question types are conditional (like pricing question types), but it will do just that. You can assign any question to only be present when given question X on the form is equal to (==) value.
