List of jobs with specific form

Hello,
I have a jobs list.
Each job is create using a specific back-office.
I need to add a button under each job "clic here if you want this job"
Then I open a page with a form (no problem)
But, now, i would like the job title included automaticly in the form.
Someone can help, give the way or steps to do that ?
