Missing Blocks after adding from the Marketplace

I have two test sites I have been tinkering with.
In one I was able to successfully add two blocks from the marketplace, AJAX Forms and AJAX Lessons.
On my second site I associated the license for these two add-on blocks from the marketplace; however, when I go to "add block" they do not show up in the list.
Any suggestions?

The AJAX Form block shows as installed; however, is not showing up in the list when "add block" is selected.
Uninstalled it, re-downloaded and re-installed it, but still not showing up.
Any suggestions would be greatly appreciated.
