I am running a centralized installation since 5.4.1.1 (currently on 5.5.1) in a Windows environment and I haven't stumbled upon any changes at all.

I use apache with vhosts and the following in httpd.conf
#BEGIN Concrete 5 centralization alias
Alias /concrete "D:/www/C5_core/Concrete5 Version 5.5.1/concrete"
php_value include_path "../C5_core/Concrete5 Version 5.5.1/"
#END Concrete 5 centralization alias

So on upgrade of core I just download the new release, pull the concrete folder out and put in the directory seen above (including the version to keep track). Then all I need to do is change the version number in httpd.conf and run the upgrade script on my sites.

I only have four sites up (two only for dev), so it might not apply to you. Just saying it's doable.
