Looking for a developer who knows Concrete5 to make a simple add-on for the website.

It's for drug/pharmacy prescriptions.

Here's what I need/the process:

- A registration form to create account and a login form to access it
- Once logged in, you can request a prescription.
- Email gets sent to person who requests and email gets sent to pharmacy/drug store
- Pharmacist then logs in to dashboard and accepts request adding what drugs the user's doctor has put on the prescription.
- The pharmacist then prepares the drug/prescription order and once complete, they go back into the dashboard and click 'Ready'. An email is then sent to user who ordered.

Quite simple really, just a combination of PHP forms adding info to the MySQL database.
