Willing to pay $50 for a quick issue resolution! HTML + eCommerce conflict

I have an HTML snippet I want to add to the Products detail area of the eCommerce add-on. When I add the code, the form shows up but the button does not function the way it's supposed to (there is a conflict with the existing eCommerce block code somewhere, most likely in the javascripts). I just need someone smarter than me to identify what the conflict is to allow this HTML to function! Thanks!

You might also want to change the title to add - SOLVED
