Default blocks failing to show up for one page type only

I have a site with multiple page types. I just went through and added some default blocks to each page type.
Creating a new page now includes the default blocks for all page types except for one; any new pages crated with this page type don't pick up the new blocks.
If I go to the page type and use "Setup on Child Pages" then it will add the block to the selected pages, but it will not add the block automatically.
Where should I start looking to fix this issue?
