Anyone know if there is a way or an add-on form that would allow the following. We have several forms that will ask a second question based on a yes/no answer. For example if yes is selected it will add another question for them to answer. If no is selected then no additional questions show up. Any idea how to do this or will it have to be done with html/php

you'll need to create an external from to achieve this. The standard form block doesn't support that.

I'm not sure of other add-ons but I've used Proforms http://www.concrete5.org/marketplace/addons/proforms/... to do that in particular.
