Rich text editor acting odd with H1, H2, H3

I've been making pages using H1 as the page title, then using H2 for topic section headings and if needed I use H3 to head sub-topics.
In the last few days when I add an H2 it published it as an H1 and somehow has changed the page title from H1 to H2. I've cleared catche and refreshed page.
Any ideas?
