Top menu and new page help?

I am trying to create a new page and have it show on the top menu. Every video i've looked at on Youtube shows only adding a page as a sub page to an existing one. i want to add a totally new page (and have it appear on the top menu) but cant find a suitable tutorial. Can someone point me to a guide for this.
