APC cache does not work correctly

If you host more than one site on a dedicated server and activate APC on all sites, this does not work.

We are using a CACHE_ID in 5.4.1 which should be unique for every concrete5 site on the box (comprised of BASE_URL and DIR_REL) - meaning that each concrete5 site running with the APC cache should have its own unique cache.
