Add a form block

Put the page in edit mode. Click on the "add to" block area for the part of the page you want to add your form to, then click the block type "Form."

 

Customize your form

Survey Name: Give your form a name that you'll remember and will distinguish it in the dashboard form results area.

Notify me by email when people submit this survey: Toggle this on if you want email notifications.

Add a New Question: 

Question: Ask your question. 
Answer type: Choose how people will be allowed to answer. If you want a free-form answer, pick a text field (for shorter answers) or a text area (for paragraph-style answers). If you want them to only choose one answer, choose radio buttons. For a drop down, choose select box. For a text list where they can choose multiple options, choose list box.

When you choose your answer type, a new field will appear -
Answer Options: These are the answers that your visitors can select. For questions with multiple possible answers, put each answer on a separate line.

When you're done, click "add question."

You can reorder the questions or edit/delete them once they're added to the area below. When you're satisfied with how the form looks, click "Add."

 

Exit edit mode

If you're satisfied with everything, click "exit edit mode" to save your changes and publish live, or preview for your self.
