take a look at this page which currently uses standard survey blocks:

I bought your block because we want to replicate this page and all its questions exactly as it is, but add a couple of questions at the bottom asking for a patient name and email address. My issue is in trying to do simple things like add a question like the ones on this page where we ask a question and they select one answer. It see,ms to keep putting the number zero by any box i add. i don't want this. i want it to look just like it does now. How do i do that? I am obviously being dumb and missing the point somewhere but i need this explained to me please. many thanks

It looks like you have multiple survey blocks on the same page. You only need 1, and then add multiple questions within the block.

Hi, thanks for respsonding. Yes currently using the very basic survey add on we do have multiple surveys - one in fact for each question.

My problem is, try as i might I am not able to figure out how to emulate even one of those questions using your add-on. I get that I can place multiple questions in one survey, but for me at least it is not at all intuitive. What I need really is instructions that explain how I set up survey questions usi9ng your add-on that look like the ones we have on the page currently, because I basically cannot figure that out

I do have step by step instructions in a video on the marketplace page:

And just click the "video" is on on the right. It goes through all the important features and will help get you up and running quickly.

The majority of my c5 addons have similar demo videos to make the learning curve as small as possible.
