Good job, easy to use, does what it says it will do. Shame there is no way to delete users other than to go into the sql db, but perhaps in the future :)

This add-on works all right and can be used for newsletter sign-ups.

But it would be nice if you could (in an easy way) create your own text fields and radio buttons, per example for name and gender. At the moment there is only a default field for the user’s email address (and the submit button of course).

On the other hand, let’s not forget, it’s for free!
