Hello,
I want to have 5 different email signup forms. So I think I need to duplicate this add on in the packages folder on my server. What do I need to change to make this possible? All signups should also be saved in an other database row.

You will need to change the package name and handle in quite a few places (off the top of my head... the package and block folder names, the package class name and handle in controller.php, the blocktype class name and database table name in blocks/email_list_signup/controller.php, the database table name in blocks/email_list_signup/db.xml... probably a table name in package-level db.xml)... basically you'll need to do a find/replace for "email_list_signup", "EmailListSignup", "Email List Signup"... probably a few more as well.
I know this answer is quite vague... it's not an easy task to accomplish (if you're not a programmer than I would recommend against it and instead use the built-in "Form" block or purchase one of the more advanced form blocks from the marketplace).
