After installing the package and adding it to a page, i get the following error sending the form:Is there an extra effort I didn't do?

I had this problem. I have fix it by uninstall package and reinstall it.

I am sorry that you had this problem, and that I didn't respond sooner (I never got a notification about this post until orange73 responded -- not sure why).
I do not know why you were getting this error, but it has something to do with the database. I've updated the package so that it will output more helpful database error messages if you have the site's Debug Level set to "Development" (Dashboard -> Sitewide Settings -> Debug).
If you run into this problem again, set your site's Debug Level to "Development" (as described above), then try it again and post the error message you get back here. I should get notified this time, but if for some reason you don't hear back from me, you can email me directly at concrete@jordanlev.com.
