This is a great Add-on if the form you require is a little complex. It does exactly what it says and works well. I have used it on a number of website and will continue to do so.

Give the basic form some much needed additions. I will be using this on every site I build from now on!

Great form builder but the UI is a little clumsy to work out who is sent emails and from where, i'd also like to be able to edit the emails that get sent from within the UI more effectively rather than having to delve into the code.

But it shows great potential, looking forward to see how its developed further for 5.5! Thanks

I love this form builder. It is easy to use and customize. The email format checking is great. I highly recommend.

The only thing I would like to see is a phone number check i.e have the user put in 5185551212 and it automatically goes to 518-555-1212 and then checks to ensure the format is correct (and they are putting in the area code). Please add this! :)

Thanks for the great plugin. Saves lots of time! Would prefer if the reports would fall under the reports of the standard form but eh, not too bad.

Making sweet, full customisable forms just became easy! This block is so easy, I love it and I can say "yes" to the customers every time!

The flexibility of this add-on is endless. Some things like the fieldsets aren't the most intuitive, but nonetheless I use this over the standard C5 form when I can.

This is way more flexible than the core form. The best feature is that you can set it to send from the email address that the user inputs into the email address field. This means that I, not being a programmer, didn't have to script anything, and my clients can easily add new forms as their needs grow. The support was prompt and helpful too.

This addon is one my of top 3 addons in c5. I am really digging the reCaptcha addition and the recipients list.

Nice gimmick often missed: The "Other?"-option in a selecbox-answers-list, that slides out an inputfield for freetext-answer in the frontend.

I have used this module for a few different projects and it keeps getting better. More functionality has been added for increased flexibility and options! Great job!

This add-on does exactly what it says it's gonna do. It's easy to style and customize. I wasted a ton of time trying to set up an aspect of a site with standard forms. I had this up and operating the way I wanted within an hour. Great value for time saved.

This is a good addon, but there is one gotcha I have discovered. It will load 180kb of tinymce javascript for the wysiwyg field whether it's used in the form or not.

I definitely needed this over the standard block. I was using an external form to take care of the functions that the standard form could not. This has taken care of that for me.

It is still a bit rough around the edges in where you have to be careful on longer forms of more than 1 of certain form fields will cause submission issues. Not a hard work around, but it does make it a little rougher.

I do wish it had more control over the look of the different fields.Be nice to have choices of text field size, or radio / check box grouping being horizontal as opposed to vertical.

However those are all minor. It is a improvement and headed in the right direction.

I love this Form Block. Incredibly flexible. I use it for online surveys, Job application forms, Sales contact forms, and your basic contact form. It's loaded with options. 5 stars!

Only thing missing is dynamic/automatic width or being able to set the width for each form independently.

Now has fieldsets for extra control of styling. You can also add a file to be sent with the confirmation email if you want to give the form filler a coupon or other file. This is a great add on and Wim's continued work on it will keep it in the front of must-haves for your website.
