Additional fields not showing in results viewer or export

Permalink Browser Info Environment
I have added a field to the extended form block, but the field does not show up in the extended form results viewer nor the results export. Can someone please tell me how to implement this?

Hi Steve,

You need to change the results page also.

The edit window does not give me any way to edit the fields :-/ Screen shot of response viewer edit options attached.

Furthermore, when I export the data the new field is present in the CSV, but contains no data
