After installing this add-on, you will be able to upload your standard PDF or Word document templates to your GroupDocs account. Once a template is uploaded, you can add fillable form fields throughout the document, such as Name, Address, Date, etc., and design a questionnaire associated with these fields. The questionnaire can be then easily published on your Concrete5 web pages. Once a user completes the questionnaire, GroupDocs will automatically incorporate the user-entered data into your document template and save it as a new, unique document.
