Not working well under 5.6.1.2

there are no options available on a 5.6.1.2 installation. See attached image.

Hello,
I'm running 5.6.1.2 and didn't see that. I've attached a screenshot. That said, could you try changing this in add.php and edit.php
<?php include($this->getBlockPath() .'/form_setup_html.php'); ?>

<?php $this->inc('form_setup_html.php'); ?>


And see how that works for you?

Attached is an updated version of Login Block, that may or may not make it to the marketplace. Feel free to check it out and see if it fixes the issue for you.

Good to hear. That version has been submitted into QA so hopefully that version or something similar should be released to the marketplace soon.
