First you have to create those user attributes, within the dashboard users & groups section. Then on the edit interface of the mailing list signup block, you can choose which attributes you want to include. It might be located within a second tab, at the top of the interface.

Got it! My confusion was that I was expecting the control to be on the
Dashboard Mailing Block Settings interface. In hindsight it does make sense
that the cite block should control the display.

That said, a little more documentation would have been helpful.
