Form Block Troubles

I am using a form submission block on my website. When a user hits the submit button, he is redirected to "my-domain.org/index.php/about-us/submit_form#formblock730" which is a blank page that never does anything. The form submission does work, but leaves the user staring at a blank page. Any idea what could be causing this?

Also, emails alerting me to new form submissions claim to be originating from the admin's (my) email address. I believe this is simply an incorrect header as I have set up a separate email for my installation of C5.7 and have confirmed that it works (the test email was "from" the correct email). There was also a report of this same problem back in December that was fixed here -https://www.concrete5.org/community/forums/5-7-discussion/form_block... but I am still seeing this problem. Maybe I just don't know how to change this?

I was able to resolve this issue by upgrading Concrete5. I don't know the version differences.
