Help with Forms

I need to create a form for team rosters on an athletic site. I'm still learning about C5 but the current form I don't see any way to accomplish my task. Ideally the form would contain a "table" type list. Any ideas?

two ways to achieve this:

1. use the external form block and create the form manually (HTML and PHP knowledge required)

2. create/modify the form block and add a new type (quite a bit of PHP and Concrete5 knowledge required)
