No Checkbox - only checkbox list

Hi
I am trying to add a single checkbox to the contact form but the options on the form only allow a checkbox list, not a single checkbox.
Do I have to mess with the PHP to get a single checkbox?

How strange, just been back in - it was making a dropdown list before but it's fine now! Theres a poltergeist in my C5!!
