Simple Contact Form Pop-Up

Looking for a way to add a simple popup contact form on a product description page with an "inquire" button. Have no problem adding the form block to the template, but would like it not to show only when the "Inquire" button is selected. Would like to do this without any redirects to another page, as I need the specific page name of the product included in the client's e-mail received without the customer need to enter the product name (same as page) when making the inquiry.

Select "Inquire" button, opens form block on same page, customer completes form, the email received by the client references the page name.

No problem with getting the form to do the job, just need the block to only appear when the "Inquire" button is selected by the customer interested in the product.

Any ideas would be greatly appreciated!

This may or may not work for you but you should check out the PugPopup in the market place (http://www.concrete5.org/marketplace/addons/pug-gallery-and-popup-utility/) I dont have it but have been looking at it for a few things, it looks pretty versatile.

secretsquirrel, Thanks for the input, looked at pug and see no indication it will work as needed with a form. Also looked at the other popup add-on which appears to be somewhat complex and based on the need to have the form reside (and refer) on the same page, and do so when creating product pages from the template, don't believe it is a choice either. Was hoping for a few code edits here and there or another possible work-around. Thanks!
