Pretty URLS

What particular magic is required to make these work? I've been through the forums and tried all the varies fixes but none of them work. I've enabled mod_rewite logging and it doesn't show any errors, I've watched the httpd error log and there's nothing but all my pages apart from my home page are 404.
I read that message but no, I'm using my own dedicated server. I've fiddled with every option I can think of in Apache to no avail.
Surely there must be a recommended config for Apache for Concrete 5?

Before turning on Pretty URL's, a blank .htaccess works for me and then I let concrete5 populate it when I flip on Pretty URL's in the dashboard.
For local development, I use a 'Uniform Server':
http://www.uniformserver.com/
Fire it up and it just works.

This is a live public facing web server. Every other site (Not Concrete 5) works just fine. I'm not convinced a blank .htaccess populated by Concrete 5 would be any different from a blank .htaccess populated by hand with the correct info.
It's extremely frustrating as I love my site on concrete 5 but this problem with pretty URLS seems to have plagued this particular CMS system since 2008. I've had this site up for 2 years and I've never got it to work with pretty URLS. It was originally a test to see if concrete was a viable alternative for the other CMS system I use but so far it's proven to be a complete failure :-(

If so, I would recommend moving your site to a different host (I'm not kidding).
