Error submitting form

I'm running 5.6.3 and have been encountering this error on form submission:

Form Processor Response: <br /><b>Fatal error</b>: Call to a member function addAnswer() on a non-object in <b>/home/arcwebacct2/public_html/packages/sixeightforms/helpers/form/processor.php</b> on line <b>240</b><br/>

JavaScript Error: SyntaxError: JSON.parse: unexpected character at line 1 column 1 of the JSON data

The form was developed by someone else some time ago and has worked in the past.

Here's the form:

I'm not too familiar with Concrete so any help would be appreciated.

Has the PHP version changed on your server?

The site is hosted on a shared GoDaddy server, so it is quite possible that something was upgraded. However, I would think they would have silos for their customers.

I will see if I can get to a command prompt to determine which version it is using unless there is a trick in the code to display the version?

Thank you.
