TinyMCE - Editing Image popup different on custom and 'simple/advanced'

It's the craziest thing. If I have TinyMCE set to "Simple" or Advanced" and I click on an image to edit, I get a pop-up editor with 3 options at the top. Great. That's what I want.
However, when I change the setting to "Custom" and try the same thing, I get a very limited version. I've attached screen shots.
I'm not much of a coder (okay, not at all, really) so I didn't want to go digging around without some guidance. It's probably something so simple I'm embarrassing myself but any suggestion would be greatly appreciated. :-)

Add "advimage" to the list of plugins on the "Rich Text Editor" page (Dashboard/Settings/Rich Text Editor)
