You should remove the columns that are empty, in your case (Description, Filters, Link).

no, it has never shown in the locations tab. i have uninstalled and reinstalled and uploaded the file and it still doesnt show. sorry for the troubles man, i can give you a password if you wanna take a look?

Anyway, I managed to reproduce it on my end as well (not sure if we got the same problem).
PHP has a default of 1000 variables that can be submitted within a form. You have 220 locations with 5 different fields (which is over 1000). You can change this default limit by updating php.ini file and updating this line to look like:
max_input_vars = 2000
Sorry I am not sure if you are a developer so if you want me to do it, PM me server login details I will do it.
