Your module has lots of potential! May I suggest the following:
- Can you add a Prev / Next buttons at the bottom so users can have a better since of what to do next.
- maybe use drop-downs in extras area to select the number of items
- A field dedicated for custom pickup / drop off or a field for place of stay while on holiday... which is good for businesses that do mostly vacation rentals.

Although I don't see the benefit of allowing the user to enter their own location. This will create a lot of work for the admin to check them all.
