CSV file import problems

I'm unable to find the sample cvs file you suggest downloading so the format is correct. Can you point me in that direction? (From the getting started page).

Thank you, With this format I get the error "too many columns"... so if I create my doc in google docs then export as .csv then import, I get this error:
An exception occurred while executing 'INSERT INTO bthwstorelocation(name, address, city, state, country, zip, phone, email, url, urltext, extra, gID) values ( 'Square B Tack','4401 County Rd 7','Florence','AL','USA','35633','256-767-5203','','' )': SQLSTATE[21S01]: Insert value list does not match column list: 1136 Column count doesn't match value count at row 1

It looks like Google docs has removed some columns. You only have 9 columns you are trying to import.
Open the csv file in text editor and review the amount of columns.

go to Import stores and on the top right you will see a button Export sample CSV
