I have also had your problems and there are two possible solutions (or maybe both as with my provider):
The first is a problem of version 8.2.1 with thumbnail creation:

So you have to set the height of 400 on file_manager_detail image size
on this page: [your-domain.com]/index.php/dashboard/system/files/thumbnails/edit/2
After that you have to rescan every image ever uploaded (since 8.2.1) in the file manager.
Maybe this tool can also help if you have many images:https://www.concrete5.org/marketplace/addons/rescan-all-files...
Also if your webhosting does not support short php-codes [<? ?>] you have to replace all [<? ] with [<?php ] in all templates.
