Form not sending question set ID

The form was not sending emails from our server. I contacted Rackspace to find out why it wasnt sending the email and they said that the application was throwing errors and that the form doesnt have a question set ID. What is wrong??

Sorry for the late reply. Been on holiday and after returning I was very busy on several projects...
Not sure what they mean with question set ID. Please check the logs for more details about sending e-mail. See if the server is giving you some kind of error...
