This addon only modifies the front-end “view” of the built-in form block… it does not have any control over what the form block does after the data is submitted.

So, the way you change the subject of the notification email is the same as you do for the built-in form block — you need to override the block controller. Copy /concrete/core/controllers/blocks/form.php to /blocks/form/controller.php, then edit the copy of that file and find this line (should be around line #460):

$mh->setSubject(t('%s Form Submission', $this->surveyName));

…change the text in there (the “%s” is a placeholder that will be filled with the form name).
