Just a suggestions...
the labels should have a "for" attribute, that way when you click the label it selects the related input. :)

looking at your code, it looks like you intended to, there's a hiccup somewhere though..I don't know php well enough to point out where.
Yes, those should be in there. With the exception of radio buttons (and maybe checkboxes?) -- the C5 form block provides those to the view in a way that makes it impossible to add labels unfortunately.
I will take a closer look to make sure it's working elsewhere though (when I have the time... may not be right away).
Thanks for bringing this up, I agree that labels with the "for" attribute set properly is important.

I've double-checked this and as far as I can tell, labels have the "for" attribute set correctly for all fields except radio lists and checkbox lists.
I'd recommend submitting a bug report to Concrete5 about the form block not using labels for those -- it's not something I can realistically fix with a custom template like this.

so, I'm using it on isitvivid.com/contact/ (and a few other sites, but I thought I'd show you here) - and even on text inputs and textareas, there's no "for"...
any thoughts?

I just fixed this with the new version (now compatible with 5.5). I also figured out how to put label tags around checkbox list and radio list items.
