I highly recommend this addon, first for its functionality. It's very flexible and the author is very willing to listen to comments and make changes for his users. Also, he was instrumental in helping me link the form results to a secure forms processing site for my client. You don't often see this level of support from developers. Kudos!
When I tried "Add element" to empty column or delete form, It's displayed "Page Not Found".
