Formidable Lite is a perfect addition for your website if you need a form block with more than 5 fields! It has great functions which make the form clean and simple (e.g. if-then). Also the support is awesome! I found a little bug, that have been fixed within a few hours and I had some problems with my SSL configuration which Wim from DeWebmakers fixed within minutes!
I'm really looking forward for the full version which offers more functions!

Although the built-in C5 fm block isn't bad, Formidible Lite is fantastic. Super easy to use, lots of functionality. The dependancy option makes it simple to modify the form on the fly based on values of other fields.
And it's a steal @ $20! I would have glady paid 2x as much.
Now if only the developer could get the full version completed and available....

I really like the formidable lite version. This can do much more than the built in form block an does it really cool:
- Good features for form feedback to sender an other defined groups
- More styling possibilities: texts and titles between form elements
- Conditional checks help you organize additional questions (they ar visibile on checking a trigger)
- Preparing forms in dashboard: no more mess with copying forms and collecting in the same database

This add-on is a long awaited form add-on for easy use in event forms. Because of the page-attributes you can add as default values in your forms, it's easy to have custom event information displayed in your form results.
Together with conditionals you can use just one form for many event pages!

Also the support is excellent, DeWebmakers fixed the non-functioning part of the add-on in no time.

I used this block to create a long and complex online Employment Application for my client and it worked very well. Depending on your theme, you may have to do some significant css work to make it format well for your needs, but all the flexibility is there to do so. I highly recommend this block for your forms. This may be a "Lite" version, but don't let that fool you as it is heavy on functionality. And their support is very responsive and helpful. Great value!

Well worth the money. It is what I need on many of the websites. Going to check out the full version. My expectations are high for that one as well!

This add-on was exactly what I needed. It gives me the options to change the sender of the enquiry form to the actual email address of the person who is completing the enquiry form.

The options to customise the email is excellent. I have been able to create formatted html emails using this add-on. The basic email form which comes with concrete5 is functional but not very pretty. This add-on gives the flexibility to organise the resulting data into whatever layout you require.

I initially had problems getting this add-on to work. But after contacting Wim (the author) everything turned out ok. I can't thank Wim enough. He sorted out the issues with my installation of concrete5. Apparently there are some issues with a core part of concrete5 which needs a manual patch. Wim did the patching for me and it's all working fine.

This add-on also adds loads of extra options to the fields which you can add to a form. e.g. The "Name" field has several options to add "Prefix", "First Name" and "Surname" or you can create custom options. Another excellent feature is the Text field which you can use Masking. An excellent feature to verify that the data being input is in the right format that you require a customer to input.

All in all this is well worth $20. And Wim goes out of his way to make sure that it's working for you as it should.

This is exactly what I needed. Very functional, easy to use.

With increasing requirements for from address and form spoofing, this add on really does the trick. In fact the "lite" version is quite robust and gives you pretty much everything you need to take forms to the next level beyond what is provided out of the box. I only needed a couple of the features it provided, but it made it much easier than programming and maintaining it myself.
