Formify Date Field Formatting Issue

I am having an issue with a date field where the styling is not correct. Sometimes is works, but many times it doesn't format correctly. The attached screenshot was taken using Firefox. It is hard for the user to select the correct date. I am using the default css for Formify. The actual form is loacted:http://www.ers-cat.com/careers/ and it is on the second page of input. I have been testing this on a c5 8.2.1 using elemental theme and I cant replicate the error in your form. Curious..Its almost as if there is some other css on the page interfering with formify but I cant see it.

I can see the issue on your site. Is it the same if you use the dropdowns rather than calendar pop-ups?
