I'm also looking for this plugin. The site currently says that it can't process payments in order to download the plugin. An ETA would be wonderful!

Finally...after MONTHS of retooling, updating, and extending Formify, it'll be back up on the marketplace this week. I'm tying a few loose ends together today and will have it back up on the marketplace as early as possible this week.

Had the feeling that the new version 8 with express forms comes close to formify. And you don't have reasons to develop it further... But it's coming back and this is good news!
