Sorry, I guess I'm an idiot but I can't get my signup form to show up in c5

Sorry, I'm not very knowledgeable about anything on the internet. So, this is probably something dumb.

I installed the MailChimp add on no problem,
Went to my mailchimp account and created a list with 3 groups
And modified the general signup form by adding two fields
Got an API Key

Opened my C5 website opened the mailchimp settings and pasted the api key
Clicked on the List button of mailchimp and saw my lists (I have 4)

Created a new page and in the main area added the mailchimp block.

The block does not have the two fields I added or my 3 Groups for a user to select.

I suspect my list is not the one being selected so I'm getting the generic form since I did not create a custom form for the others. But if this is the case, I can't find a way to specify the list I want to use. In the window with all the lists, I couldn't find a way to make the list I want the one that is used for my subscribe page. And I couldn't find any place to paste the list ID to make it the default.

Any ideas what I'm missing?

Oops sorry, I found where I specify the list...actually guess I forgot it was there when I added the MailChimp block to the page but just saw it.

So, I do have the correct list but my MailChimp general form with the two extra fields and the 3 groups are not displaying in c5.
