I'm getting the following error on submission either with or without review:
Fatal error: Call to undefined function mcrypt_encrypt() in /var/www/vhosts/theperformancepredictors.com/httpdocs/packages/proforms/blocks/proforms_display/controller.php on line 135

mcrypt_encrypt is a standard php function. You need to ask your web-server to enable encrypt.

Would have been nice to know that before I purchased it.

I've since gone with another form add-on.
