Can't add values for dropdown selection using "Select Values" question type

I can't add values to the "Select Values" question type. The Add button doesn't work. I've tried different browsers, etc. See attached screen-shot.

It looks like the issue is only if you try to add the values in the question edit popup within the form "Edit" window.

I was able to edit the question directly with the + symbol, then I was able to add the values (see screen-shot).

I did a double check to see if the "allow multiple values" was checked first; yes in both cases.

I'll try to edit the question type directly before adding in the form editor from now on.

One more issue: The "Preview Form" did not work; it worked yesterday, but I've added more questions. Is there a limit to the number of questions before the Preview pop-up doesn't display (blank page).

definitely not seeing this. My guess would be that you have a javascript error in your site caused by an addon. If you right click and "inspect" the console tab while trying to add/edit the select option, you might find a clue there.

I looked on the js console for errors; didn't see any.
Now I can't use the editor at all!
The problem got worse after I posted this question because there was an update for this plugin that loaded and now I get this error message when I click in the 'Example Form" edit-icon:
Parse error: syntax error, unexpected T_PAAMAYIM_NEKUDOTAYIM in /home1/nlecom/public_html/wifilibrary/packages/proforms/models/attribute/types/associate_object/controller.php on line 128.

This is what the console says on that error:
The character encoding of the HTML document was not declared. The document will render with garbled text in some browser configurations if the document contains characters from outside the US-ASCII range. The character encoding of the page must be declared in the document or in the transfer protocol. 3

Thanks Chad, I believe you found my mistake in the other message thread @stephaniemull. The PHP version was not 5.3+ because I was installing on an old site as an add-on domain. I've upgraded PHP to 5.4.
