Issue: Review Form Entry

Based on the current demo installation athttp://demo.chadstrat.com
I have found these issues within "Review Form Entry"
1. If you submit the form WITH the "Enable Review" (Form will not be submitted until reviewed.) checked .. then some of the values shown within "Review Form Entry" reports get wrong. In particular: "price select" and "price total" inputs go back to default and 0.00 value.
2. Within "Review Form Entry" report, If you try to edit a record by clicking "Enter edit Mode" .. then the values there changes. In particular: "price select" and "price total" inputs go back to default and 0.00 value.
I have a 15 seconds ScreenCast for this last Bug:

Please let me know as I like a lot this add-on and I really would like to buy it.
