Select Question Type W/User Input

Hey all,

So I've been working on a website for a military museum here and they want people to select what war/era they were involved in on this form. I was using the Select Question Type to accomplish this (which works out well) except they want an Other category which a person could check and then fill in the blank.

My issue is that when I click the box to allow a user to add to the list, my list doesn't display on the form. Anybody know how to work around this or a better way for displaying it?
