Would be great to have the ability to add sub categories and difficulty levels to questions.

We'd also like this feature, where a user must pass one quiz before they can move on to the next. Could you provide an estimate for this customisation?

We can do the customization of the product if you interested. Also if it will be fine with you we can add it to the public version so it will be supported further. Improvements looks ok for me.
We do not plan to upgrade the product at this stage so this is all i can offer so far.
